nope, that was it, apart from "If its ok to kill one person in the second 2 scenarios to save 5, why is it not ok to kill Bill?"

Good point. However the second two situations are extreme under pressure, almost split second decisions. If you don't kill one then everyone dies. Where as with Bill if you don't kill him then the patients waiting for organ donars can continue to wait and hopefully get a match soon.
